Apple Health is Washington's Medicaid program. The Health Care Authority writes the benefit. Your managed care plan picks which outpatient clinics it will authorize and pay. People comparing apple health rehab washington options around Seattle or South King County should run those two checks first.
We treat adults in outpatient care only. Partial Hospitalization, Intensive Outpatient, standard Outpatient, and a Virtual Intensive Outpatient Program. We do not run detox, inpatient hospital beds, or a VA program. If you need medical withdrawal management first, say so on the call so the team can send you to the right next setting.
Coverage is not automatic for every clinic or every weekly schedule.
Washington still runs medical-necessity review. Plans can demand an assessment, a diagnosis, and a level-of-care note before they pay a claim. Your managed care organization writes that determination.
That list is clinical capacity, not a pre-approval from Apple Health at this clinic. Authorization beats assumption.
Your member card names a managed care organization. That plan holds the provider contracts. If a Seattle clinic is out of network for that plan, Apple Health usually will not pay the claim. The service type can still be a covered benefit.

PHP is the highest outpatient intensity we run. You return home or to sober living at night. IOP is the usual fit if you are going back to a job or classes and still need several clinical days. OP is weekly maintenance. Virtual IOP is for people in Washington who can do structured outpatient work remotely.
That schedule only matters if the payer funds the level you actually need.

Apple Health is Medicaid, a public plan with its own managed care contracts. Commercial PPO rules do not apply to it. Our PPO list does not prove that an Apple Health plan will pay us. We will not promise that any plan, Medicaid or commercial, will cover your episode of care.
If you have both Apple Health and a PPO, say both names on the call. Coordination of benefits can change who pays first and what you owe.

Virtual IOP on Apple Health is a plan-by-plan decision. Telehealth rules and prior authorization decide it. Get the answer in writing from the MCO before you treat remote care as paid.
Ask the plan for the denial reason in writing, plus which level it will authorize instead. You can appeal through the plan and the HCA process. You can also ask the plan to name an in-network clinic.
